چکیده

Introduction Lung protective ventilation, LPV, is recommended for ever wider patient populations, even those without primary lung disease. Low tidal volume ventilation is a first-hand remedy for LPV that is facilitated by dead space reduction. Inspiratory flow pattern affects gas exchange. In ARDS patients, it was recently shown that a long period during which inspired tidal gas is in contact with resident alveolar gas, mean distribution time, MDT, as well as a high end inspiratory flow, EIF, immediately before interruption of inspiration augment exchange of C02 by reduction of airway and alveolar dead space.

An appropriate inspiratory flow pattern can enhance CO2 exchange, facilitating protective ventilation of healthy lungs.

BACKGROUND In acute lung injury, CO2 exchange is enhanced by prolonging the volume-weighted mean time for fresh gas to mix with resident alveolar gas, denoted mean distribution time (MDT), and by increasing the flow rate immediately before inspiratory flow interruption, end-inspiratory flow (EIF). The objective was to study these effects in human subjects without lung disease and to analyse the...

Dead space and CO2 elimination related to pattern of inspiratory gas delivery in ARDS patients

INTRODUCTION The inspiratory flow pattern influences CO₂ elimination by affecting the time the tidal volume remains resident in alveoli. This time is expressed in terms of mean distribution time (MDT), which is the time available for distribution and diffusion of inspired tidal gas within resident alveolar gas. In healthy and sick pigs, abrupt cessation of inspiratory flow (that is, high end-in...

Effects of lung protective mechanical ventilation associated with permissive respiratory acidosis on regional extra-pulmonary blood flow in experimental ARDS

BACKGROUND Lung protective mechanical ventilation with limited peak inspiratory pressure has been shown to affect cardiac output in patients with ARDS. However, little is known about the impact of lung protective mechanical ventilation on regional perfusion, especially when associated with moderate permissive respiratory acidosis. We hypothesized that lung protective mechanical ventilation with...

Protective strategies for one-lung ventilation

provided the original work is properly cited. CC Hypoxemia and acute lung injury (ALI) are major concerns with one-lung ventilation (OLV) during thoracic surgery. Hypoxemia is usually the result of alveolar hypoventilation and an increasing shunt fraction, whereas ALI is caused by ventilatory stress (volutrauma, atelectrauma, and barotrauma), reexpansion pulmonary injury, and the lung surgery i...

Original Contributions Work of Breathing During Lung-Protective Ventilation in Patients With Acute Lung Injury and Acute Respiratory Distress Syndrome: A Comparison Between Volume and Pressure-Regulated Breathing Modes

BACKGROUND: Pressure-control ventilation (PCV) and pressure-regulated volume-control (PRVC) ventilation are used during lung-protective ventilation because the high, variable, peak inspiratory flow rate (V̇I) may reduce patient work of breathing (WOB) more than the fixed V̇I of volume-control ventilation (VCV). Patient-triggered breaths during PCV and PRVC may result in excessive tidal volume (VT...
